ok so how does it work, u get really hurt and u need to be airlifted how would they know you have the coverage and to get the right airlift?

Seriously, they only worry about transferring you first, billing you second. So you have time to wake up and let them know

does that mean i could get it and since all of the careflight would be "out of pocket" they would cover it?

ACTUALLY, You can sign up the day before you want coverage.
Money order receipt is proof of coverage.  There is no reason to wait that long, though.
